Dr. Kozak is truly of a dying breed. What I mean is that he will actually address your problems and treat you without referring you to a specialist each time you present a different concern. If he thinks you need a specialist, of course he would refer you to one, but he doesn't run you around & get different "specialists" involved when it is something he can easily handle on his own. He's precise and straight to the point. He explains his diagnosis and treatment orders in a way that each individual patient can easily and fully understand. There is nothing worse than waiting a week to see your PCP, only to have them refer you to someone else. I don't need a manager that sends me off to someone else each time I pay them a visit. I need a doctor that treats me and that's what he does. That's why I chose him as my doctor. He always makes time for you when you really need to see a doctor. Amazing personal doctor that's very informative,explanative of all approaches in your choices and his suggestions for your treatment and recovery of different aspects of your well-being!!! 5 stars out of 5!!! Would give him 6 stars if it was an option!!! Staff goes above and beyond as well to care for your needs and very informative of any personal changes, well being as well as all collection of vitals, changes or additions to meds, friendly and understand in cooperation of upcoming appointments/and or emergency visits made available, should any other medical hardship arise prior to your next scheduled follow up visit!!!! 5 stars to the staff! ???? Dr. Kozak, personally, is the first doctor to explain to me what’s going on, the process and why he’s prescribing me what he is. He breaks it down until you understand and are comfortable. None of my questions or concerns were left unanswered. Side note: he had an awesome attitude the whole time! So thankful!

If you are looking for a doctor that doesn't pull punches, cares more about your health that your weight and has a sense of humor, Dr. Kozak may be right for you.

Sadly my uncle has been going to Dr. Kozak for some time. My uncles mental health is not good. I drive in from pittsburgh to connesville to help tens to him. I took him to a doctors appt and was treated by Dr. Kozak like first I did not belong there, not to mention his very rude demeanor. He spoke to me in a manor as to tell me he did not have time to address concerns because and I quote: " I only meet with my patients for 10 minutes." Excuse me! Where I come from your seen and treated accordingly. Not to a 10 minute slot. The nice thing about all the millions of doctors in the world we do not have to deal with him any longer . I went to see the doctor for a referral for In home health care for my uncle. I specifically told the doctor i needed to sign release forms as well as forms to have the doctor talk to me in regards to michael in his condition and he said we dont keep those types of forms!!!! What???? Someone help. I ask that my contact number be the main contact and when referring the nurses for home health care, they need to call me to schedule appointments so I can travel in from pittsburgh to be there to meet with them to introduce my uncle to them slowly since hes scared to death of everything and every body. He is truly an amazing doctor. He actually cares about his patients. His staff is awesome . My kids have been going to him since they were little and my oldest still sees him and she adores him.

Dr. Kozak is the best family physician I have ever had! He doesn't rush you in and out, and takes the time to listen and explain things to you. His bedside manner is excellent! His staff is amazing, they are always nice and friendly! You don't feel like a number in this practice, you are treated like a friend!

Dr Kozak is a very caring and dedicated doctor. I have been seeing him since he first began his practice and wouldn't have any other doctor. I have referred many friends and relatives to him, knowing they are in good hands.

He is a excellent doctor. Listens to answers all questions a man of good charaacter

There’s a lady I take care of and she adores Dr. Kozak. He is really good with her, he not only makes the booboos go away but he sits and talks with her about different things that’s bothering her.
