Dr. Michael Ham is accused of fraudulently billing several government institutions and private health insurers for hundreds of thousands of dollars for nearly a decade, according to the Arizona Attorney General’s Office. He was arrested on Thursday, Jan. 2, in Prescott Valley, where he has been practicing at Kokopelli Eye Institute.

I was genuinely impressed with Dr. Ham's manners and abilities. He was not only personable and engaging prior to my cataract surgery in May and June of 2019, but on the day of my surgery he answered a few of my lingering questions, and afterwards for post-op follow-up services, his demeanor was consistent. He was skilled and showed expertise in his area of medical profession.

The exam went fine but 2016 & older reading material in your waiting room is a disgrace. The waiting time is at least 30 minutes plus & that is normal. Everyone there who works there is very nice . The problem is I would not mind so much the wait if there was something at least interesting on that expensive but bad TV or some DECENT CURRENT YEAR magazines not 2016 which is what I picked up (Home & Garden book) or older to read. Which I put right back down too. What does it hurt to bring in some decent reading & CURRENT YEAR magazines. Are you doctors really that broke....I find this in other doctors offices as well...or does anyone really care what your clientele think. But yet you want us to believe your excuse's like what I was told today here that when Dr. --- is here we are always behind. Now this was honest and funny all at the same time or another one said "oh we had 2 emergencies that is why we are behind........This I do not go for.....The first one was better.......This is a routine line to just flat lie to us patients and you tell us to arrive early......I arrived 20 minutes early and still I did not get in until I had set there at total of 40 minutes. The truth is always explainable & believable to patients but lying is never exceptable in any case. I still like the first reason best it was so natural and up-front and honest...... Dr Ham and his staff at Kokopelli Eye Institute are very kind, explain things well, I did not have to wait for long, were excellent with the pre-op instuctions and after care for my cataract surgery. It was all a very positive experience and I would highly recommend Dr Ham.

Recently had cataract surgery. They didn't tell me they would be covering my entire face with a sheet of plastic and when they did, I felt like I was smothering. When I tried to explain that I felt like I couldn't breathe, Dr Ham yelled at me! No one explained anything prior to the surgery about what to expect. There were no pre-op videos or any information. Nurses were all extremely nice, but Dr Ham has no compassionate bedside manor. Surgery was successful, but terrifying to me.

Just when I was truly despondent about medical care, Dr. Ham stepped up. I had trouble getting anyone to see my husband, even a visit to the ER. Dr H's staff worked him in, Ham did everything he could and took the time necessary to actually help us in a serious situation. The whole staff was willing to stay late. I cannot say enough good about his help. For what it is worth, I am a retired medical professional, so honestly not the easiest to please.

Professional. Courteous. Very efficient. Kokopelli Eye Institute is an exceptional enterprise. I used both Prescott and Prescott Valley. Both were equally exceptional.

Very pleased with the service I have received. Will definitely recommend Dr. Ham.

Dr. Michael Ham has been our family eye care provider for 15 years. While people say cataract surgery is "a piece of cake", it is still scary for the person having it for the first time. Dr. Ham and his entire staff made the procedure not so scary and ensured that all my concerns were addressed.

Dr. Ham has helped me with a unique problem. I contracted shingles in the right quadrant of my face 12+ years ago. The outbreak affected my right cornea which left me with extremely poor vision in that eye. Dr. Ham was recommended by a cornea specialist in Scottsdale, Arizona. For the past 12+ years I have been grateful for that referral and the care I have received from Dr. Ham at Kokopelli Eye Care. I recently had cataract surgery by Dr. Ham which helped the vision in my left eye greatly.

The surgeries went fine, was not even aware of the first eye surgery happening.

The staff and Doctor answered all of my questions and concerns before surgery. Thank you for making me feel more at ease. -Connie R

Dr. Ham is an excellent doctor, but needs better office organizational management. I have had to wait numerous times an hour, and I have been seeing him for years. Emergencies happen, but if there are this many, appointment scheduling needs to reflect this frequent possibility. Patients should not have to wait this long on a regular basis. My appt was at 8:20 am today, Mar 19. How could Dr Ham be an hour behind at this early time? (No explanation was given.). Due to other appts, I had to l

Dr. Ham performed surgery to remove cataracts in both eyes. He was able to measure the degree of astigmatism and put in corrective lenses as well as do corrective measures in both eyes. After wearing glasses since age 12, I can now see clearly within two days of my second surgery without glasses. While he had hoped to lessen my astigmatism, neither one of us can believe the degree of success achieved.

I Have been seeing Dr Ham for all of my eyes issues for many years. Each and every time he has been right on with his diagnosis and solution. He is patient and very through with his explanations. I would recommend him for anything to do with your eyes.

Dr Ham is an excellent physician! He is so concerned about helping you achieve the best possible vision . It is truly a wonderful feeling to be in the care of such a caring and gifted Doctor!

My first experience with this office went very well. My last experience was completely the opposite. I paid 600.00 for eyeglasses that are crooked and the right lens is not completely in the frame. No one looks good in crooked glasses by at least 1/4 inch and his office refuses to fix them. So I am back to my old glasses which are straight. The optical/sales person tried to me my "ears are crooked". Hmm but not the case with my old glssses. Dr Hamm should be more involved in customer service

Dr. Ham is a well-respected and good surgeon but should do something about the absurdly long wait times for patients. It appears it's just become routine to wait upto 2 hours. This doesn't work for me. (Surgery scheduling is more efficient.)My time is valuable and I don't feel it's respected.

The staff are friendly and courteous. Although I feel the office over schedules to many patients. And my wait times were 2 and 3 hours long. Which leaves the doctor little time with patients. Overall my care was good, but the wait times I do not want to experience again.

I appreciate the care Dr Hamm and all of the staff provided me. My first eye is done and second in 4 weeks no pain answered all my questions If I had three eyes I would get the third one done with him also Only a talented few could do what he does

Excellent. I've seen some of the very best opthalmologists in southern California and can guarantee any patient that Dr. Ham is hands-down probably right up there with the very best of them (if not better). What a surprise to move to a small community like Prescott Valley and find a physician with his knowledge, temperment and qualifications.

While I am fairly confident that the examination and diagnosis were correct, I am unhappy with the general demeanor of Dr. Ham and his staff. I was treated very impersonally and never received one smile from either Dr. Ham nor even one of the four staff persons with whom I interfaced. I had the distinct feeling that I was being processed through a production line. Even the seating in the waiting area is set up with rows of chairs facing a TV screen - reminding me of a budget theater.

Pleasant, knowledgeable, efficient, professional. Listened to my concerns and needs, and adjusted treatment accordingly.

Dr. Ham and his staff were very professional. They answered all my questions and made me feel confident. I am a big chicken and the way they have treated me made me feel at ease. I have told many people it was easy. I felt no pain. When I went for the next eye I was very calm and not afraid at all. Thank you, I can see VERY well. Thank You, Mary E.

I?m a retired Director of Management Consulting from one of the largest consulting corporation in the world. You can usually tell the health of an organization by its people, processes, professionalism, and facilities. I have used Dr. Ham?s services for my annual eye exams and just recently for cataract surgery. He and his people are skilled professionals that use well thought out processes and facilities to meet their clients? needs and expectations. They do excellent work!

Dr. Ham was very personal, honest and caring. The operation takes about 5 minutes and was a wonderful experience. He gave me 20/20 in my right eye and two days ago did my left eye for close up vision. I now have mono vision and love it. An Excellent Dr. who cares about doing the best possible job obtainable. Dr. Ham removed and changed the lens on both my eyes a month apart because of cararacts. I was given detailed written instructions for before and after the two surgeries with follow up visits the next day and a week later. Everything was organized and clearly explained, so that I felt very comfortable with the procedures. I now have new glasses and can see clearly with sharp vision. I could not be more satisfied.

The Dr. always greets you with a hand shake and looks you in the eyes. He is pleasant but gets right to business with the exam. I ran into some difficulty and he gave me options and guided be through my decision. I was pleased with the outcome and would recommend him whole heartily.

Dr. Hamm was researched by me prior to him performing surgery on both my cataracts. My experience has been terrific. No complications and my vision and color perception vastly improved. I would recommend him to any of my family members. He displayed great interest towards myself and the procedure. If you required his services you will be more than pleased.

Dr. Ham was short and to the point. The first wait for left eye surgery was in Prescott Valley, very little waiting. The second wait for right eye was in Prescott, about 45 min. wait (had to park on the street) both surgeries were quick, both eyes are clear and I don't have to wear glasses after 60 yrs.

I explained my problem to Dr. Ham. He told me it was cataracts. Surgery was necessary. He patiently answered my questions. There was no problem with the right eye. One month later I had the left eye done. Surgery went well. The next morning I went in for the 24-hour check up. I had difficulty reading the chart. I was frantic! Dr. Ham examined the eye. The cornea was swollen. He instructed me to continue with the drops and all would be fine. Three cheers for Dr. Ham. I could see!
