Jason David Eubanks

Dr Eubanks has performed three surgeries on me, all have failed. After my last back surgery , was in constant pain and new something was wrong.. he had me get a CT, revised it and said fusion looked great, all healed. After consulting another surgeon and him reviewing, a complete 180.. fusion was a mess. I had to have revision surgery and everything taken out/replaced. This was just in a matter of months of his handy work. Poor disposition and arrogant. Would highly NOT recommend.

Hello to all. I would like to Thank everyone for the wonderful care that I received leading up to and continuing thru my recovery. I HAD AN EXCELLENT EXPERIENCE AND OUTCOME WITH THE SURGERY PERFORMED BY DR. EUBANKS. HE IS A FIRST CLASS SURGEON.

Had major surgery to lower back. Had excellent results-he and his pa were thorough and professional-my stenosis issue resolved

It's been 4 weeks today. Six plus hour surgery by Dr. Eubanks and his team. The surgery consisted of combination of laminectomy, spinal fusion and a few other pieces and parts in a 16" incision, never felt a thing and I feel like a million bucks now, maybe more! I have had almost no pain from the surgery, this surprises all of us, and am now walking, part time with a cane, which I believe by end of week will not be needed. Its 4 weeks from surgery, I am way ahead of where I was before surgery. Pre op meeting with Dr. Eubanks and Michal Duller, PA-C who explained everything, answered all my questions fully and I definitely felt relaxed about the upcoming surgery. The follow thru after surgery has been excellent along with an in-home team of physical therapist, nurse and occupational therapist. A 100% recommendation to Dr. Eubanks for his expertise and the people he has put on his team.

Dr. Ubanks preformed back surgery on me and the same day I had releaf of constant pain in my leg and my foot. He is very great to his patients, explains everything that is going to happen and answers All your questions. I would highly recommend him.

Dr. Eubanks and his staff are all very courteous and professional. I got opinions from 3 other physicians before making the decision to have surgery. After exhausting all non-surgical options, I went with Dr. Eubanks because he offered a minimally invasive approach which meant a shorter and less painful recovery. I am only 3 weeks post-op and the nerve pain that I had before the surgery is gone. Michal, his PA and Dawn his secretary were both very responsive with everything from scheduling the surgery to answering my many questions and getting prescriptions refilled. I am very happy with the outcome and would highly recommend him.

Due to covid outbreak in office my in office appointment was cancelled and was scheduled to have phone appointment. My scheduled phone time came and went with no call. I waited 3 hours and I finally had to call them. Was told my appointment was somehow misplaced but that the Dr would call me. Dr Eubanks finally calls the call lasted about 5 minutes. Asked how I was doing I told him my problems he asked what I do for that and that was it. Nothing to help me. Doesn't mention what to do for the multiple herniated discs or the broken lower back I have. Nothing to say about the huge bulging lump on my neck. Didn't say anything about how my lower back might be contributing to my hip pain or anything. Just tells me nothing he can do and hangs up. Wow thanks a lot for your caring and great bedside manner. I know it was over the phone and sorry to inconvenience your office with having you call me instead of seeing me in person and losing my appointment in the process and all but don't take that out on your patients. Did you even look at my x-rays? Maybe it is just discrimination because I have Medicaid insurance? Who knows and now I have to find yet another Dr and wait to be seen again... Eubanks is dry, no personality. His office staff is the worst I've ever dealt with, just rude and inconsiderate.. Dawn, who is responsible for getting prescriptions refilled can't even do her job. Both Walgreens and myself called the office numerous times to get a simple lidocaine patch refilled. This went on for a week and still never got filled. I am very satisfied with Doctor Eubanks and all of the staff.

He was compassionate and understanding of the pain I was in and helped me find the diseases that was cause it. He did an amazing surgery and recovery process. He was with me every step of the way. If anyone ever asks me for an orthopedic recommendation I will refer them to Dr. Eubanks!

Where to begin. The staff are excellent, respectful and helpful. The PA (Michal) is attentive to your needs, she answered all my questions, called me back, and handled everything professionally and expertly. Dr. Eubanks is great. Very friendly, courteous, and most important .. my pain is gone. All were very concerned and interested in my health.

Amazing experience from scheduling my appointment to my initial appointment with Dr. Eubanks to my surgery. Everyone at UH was very friendly and helpful. I have never experienced such thoroughness from having x-rays done prior to my first appointment, the PA calling me before my surgery to verify my medical issues and to reassure me, how everyone was so efficient and knowledgeable the day of surgery, to my followup appointment after surgery. This was my first experience with UH and very impressed. Dr. Eubanks explained everything and made me feel very comfortable. Enough so that after putting off surgery for a year and a half I finally had it done and am thankful that I did.

Dr. Eubanks solved the low back pain, and pain down my leg that had kept me in bed for 7+ weeks. I simple discectomy and I am 100% better! He was professional and courteous, took time to answer my questions and I never felt rushed.

I had a ruptured disc in my lower back with a great deal of pain. Dr. Eubanks explained my problem and answered my questions succinctly during the consultation when it was decided I would have surgery. He and his staff (especially Mikael) were very professional and reassuring on the day of the surgery, which was very successful. I've been having issues with losing strength in my hands/arms. After just looking at the x-ray Dr. Eubanks knew what the problem was. The visit was fast and friendly, the Dr. and his staff could not have been better. You can tell right from the beginning, they care and have the expertise to help you. I left the initial visit feeling like I would finally get back what I had lost. His PA Michal is extremely professional and attentive to your needs as a patient (and very friendly, you can really tell she cares). I highly recommend Dr. Eubanks and His PA Michal Duller. Dr Eubanks is extremely knowledgeable and honestly cares about his patients. I brought in my MRI and he pretty much pin pointed my condition but requested I do another Xray being that mine did not show a side view witch he did in his office. After he looked at my Xray Dr Eubanks told me exactly what I needed done and we scheduled for the following Friday. Best decision I ever made after suffering two years with excruciating back pain and sciatc pain I am 100% sciatica and pain free. Had my first back surgery over twenty five years ago & didn’t think I would ever find another great surgeon, but wow did I. He reviewed my MRI let me know what I would need done & told me whenever I was ready he would be there. Never any pressure, after many second & third opinions I knew he was the right one. It has been four months since the surgery & I already feel great. I recommend anyone thinking of surgery go see him he will give all the information to make an informed decision. His entire staff, Dawn his secretary & Michal his PA are equally phenomenal, I am sure I drove them nuts with questions but they were great about it.

I was in misery with lower back pain and sciatica for 10 years. Tried many types of treatments and nothing helped. After one appointment with Dr Eubanks knew immediately and help me understand what need to be done. He performed a TLIF Surgery on my back. I came home the same day. Dr. Eubanks and his staff, Michael Duller and Dawn Howard, PA-C,  were excellent throughout the whole process, they were very helpful, kind, and treated me very professionally. My surgery went extremely well. After five weeks I can honestly say I received a miracle! Lower back, hip and leg pain were reduced significantly. Little nerve pain left and which is getting better by the day.  I have been back to work and feel like a new person.  Dr. Eubanks is an absolutely highly recommended and trusted Orthopaedic doctor.  He is also kind, caring and very respectful. Operated on my husband in early 2018 after he fell down the steps. Went into surgery in pain but could move, came out partially paralyzed. His cage (the metal that was put on the spine) loosened up. At one month follow up, they saw the screw was loose but didn't monitor it. It got looser and looser causing more pain. My husband went to a neurosurgeon in NY and underwent another surgery to correct the first one. That surgeon was surprised by the condition he was left in after the first surgery. My husband still doesn't have full use of his right arm but is more improved and less pain due to the second surgery. Be careful if you have to have back surgery. Don't just take referrals from your family doctor. They just refer to the ones in their system and don't research to find most effective for the patient's issues.

The staff is very knowledgeable and helpful. They made me feel comfortable, and are very reassuring. They did an amazing job on my surgery, and took great care of me for the pre and post operation. I am very thankful for my care team

I recommend him to anyone? I've had 2 neck surgeries since 2012 and I'd never let anyone but Dr. Eubanks operate on me. He's kind,caring and has great Bedside manors. He treats his patient like Family and he is just amazing. Great Dr. Id give him 100 stars!

Everything from first appointment to surgery to release handled very well by Dr Eubanks and staff. Think pa Duller was great.

I had lumbar spine surgery 3 cage scoliosis pins and rods , in one day i had the surgery and was sent home the same day . Nurses where in such a hurry to get me out of there they were flip flopping me around to get my clothes on me they didnt put my bra on me, they plopped me in a wheelchair and sent me on my way home which was an hr away, only spent 2 hrs in recovery. While my boyfriend was driving me home he gets a call from Dr. Eubanks stating I will be in recovery for another hr then will be ready to go home. When in fact I was in the seat next to my boyfriend. Dr Eubanks had no idea where I was, to top it off I was sent home with a pain med that did not exist ( percocet 10/300 ) that night after major back surgery i was giving Tylenol 3 , I had to go back to ahuja an hr away from where I live to get correct prescription after having major back surgery not 12 hrs before. Went back 3 weeks after for follow up visit where they explained to me what they had done. The physicians assistant removed the outside stitches on the back so I thought, I have felt and thought she might have left some in. I could physically feel them they hurt very badly, yesterday 10/23/18 a month and a half after surgery i went to personal physician had him look at incisions on back and he believes stitches where left in in a couple of the incisions, referred me to a surgeon Dr Kim at Ashtabula Clinic to have removed, which i will see tomorrow 10/25/18 at 4:30 pm. Needless to say my experience with Dr Eubanks and staff is something of less desirability, and I will be speaking with an attorney. I went to see Dr. Kim he in fact removed 2 stiches from incisions in two spots on back i have 7 incision sights on back , i believe there are two other incision spots that have stitches still in there but are now under scar tissue. He thinks I need to return to see operative surgeon to discuss the situation ,not sure of that. I had lower back surgery and they broke off a piece off an instrument in me and then had to leave it in there, now i can never have another MRI dine because there's a piece off metal stuck in my back.

I went from being in tears every day to having a big smile every day! After months of trying chiropractic care and PT I finally made the decision to have the microdiscectomy procedure and I am so very thankful to Dr. Eubanks and Michal Duller who has returned every phone call I have made to her :-) I sincerely recommend them if you are looking for pain relief!

Had severe pain for two years from a bulging disc and stenosis. Tried PT and pain shots. Was very reluctant to try surgery to say the least. Dr. Eubanks performed a minimally invasive lumbar micro discectomy and lamincectomy. Home the same day. Back to work in a week. After four weeks I can honestly say I received a miracle! Dr. Eubanks, Michal Duller, PA-C, and staff were wonderful! I was treated with TLC. After getting a few options for my surgery, Dr. Eubanks was able to preform my surgery as an outpatient. I was able to recover at home, which was very important to me. I would highly recommend the the CLE Spine Center if you needed back surgery.

I am three (3) weeks out from laminectomy surgery. I no longer have shooting pains in my legs. I am able to walk upright without pain. I am currently walking 1.5 to 2.0 miles a day. The surgery was a little more extensive than Dr. Eubanks anticipated but everything went well. I am feeling very good about the procedure and outcome.

I give Dr. Eubanks and his team credit for returning me to my normal self after many months of painful disability. In addition, the entire process leading up to my recovery was gratifying. I give highest marks for competance and compassion and would like to commend the hospital (Ahuja) and surgical team as well.

he did my neck fusion and I healed fast and I would not trust anyone else to operate on my my spine. I have to have lumbar surgery next so Gerri g ready to make my appt best surgeon around!!

I can not begin to thank Dr EUBANKS enough. I had my surgery in November and had immediately relief with in two weeks on my right side.Im back to work and never felt better thank you so much.

Dr. Eubanks, explained my procedure, to where I was able to understand what was going to be done and the recovery. His assistant was fantastic, she also listened to what I was saying. When I need to speak to them, my call was returned really quickly. I have to have another surgery And I will go and have him do it

When I talked with Dr. Eubanks initially he said that some of his patients went home the same day or the next day. He never said there could be complications. I had given him a C D of 2 MRI's plus he had X-rays done. Well I WALKED in, had surgery & was CARRIED out. I've had 8 months of therapy. I still have to use a walker & wheelchair to get around. Not what I expected.

Didnt help me at all. Sent me along with no follow up on a chronic problem. Disappointed.

DR EUBANKS HAVE DID4 BACK SUGERY ON ME NOW IM GETTING READY TO HAVE MY FIRST NECK SUGERY DONE IN 2 WEEKS, I MUST SAY DR EUBANKS HAVE BEEN SO GREAT TO ME ALL OF MY PAIN IS GONE FROM MY LEGS HE HAS BEEN BY MYSIDE EVER STEP OF THE WAY WITH ME HE HAS ALSO GIVING MY LIFE BACK TO ME HE IS THE GREATEST.

Rumored to be a great surgeon, highly intelligent man with the mantality of a 10 year old. Fancy clothes, expensive watches and shows distain for his patients. If your injury was worker's comp related he is quite the politician.

Worst Doctor that I have ever been to in my life. He was more concerned with getting out of the office and getting to the next patient than offering any treatment solutions or additional remedies. Really didn't want to answer any questions that I had either. Don't waste your time, money or insurance on this Doctor. He's more concerned with his wallet than patient well being and care.
