Dr. Daniel Richard Fields

I received a very unsatisfactory interaction in November 2018 with Dr. Fields. His interactions were rude, sarcastic, condescending, and threatening of court. He is unwilling to work with myself once I was notified of a claim that was never submitted to my insurance. The date of service was three years old. An attempt to resolve the bill was made which Fields refused, he was uncivil and discourteous. An attempt to resolve the matter should be made prior to court notification. In his defense, I had a change of address during the time. However, I was never notified of a bill while receiving treatment. Dr. Fields left a note on my car.
