I went to Dr Mccollum office to conduct a Minnesota multiphasic personality inventory-2 test. I was informed the payment was $100 Cash only. upon entering the practice and patient information forms were notably dated. Within the patient information form in all the captions that asked for social security number I denoted the last four of such. Further this form asks for my address, date of birth and telephone number (credit profile) . I was informed that my entire social security number must be written in all the captions that asked for it. I ask what happens with these forms with all of this sensitive info? It was expressed to me that it sat in a box for ten years then it was professionally destroyed. I asked if it was possible my entire number could not be memorialized on paper and expressed I would be ok if it was stored on their computer.
