I spent 1.5 hours filling out a self-evaluation. The report indicated that I scored moderately high for ADHD. For Bi-Polar, the only indication was that I talk fast. Dr. Johnson felt that because I talk fast, I must have Bi-Polar disease, despite absolutely no evidence, even after being asked many questions verbally related to this besides talking fast at times. When I asked him why he made this diagnosis, he simply said because I think so. Self eval is useful, but take to someone else.
