Michael S Irwig

Finding a Physician willing and able to prescribe hormone replacement therapy for transgender people is hard enough. Finding such a Physician with a profound understanding of the numerous medical, physiological, psychological and sociological issues involved in HRT is nearly impossible. Dr. Irwig such a person. He’s the only such person I know of.

In August of 2023, I saw Dr. Michael Irwig in Endocrinology at BIDMC. I feel like Dr. Irwig not only wasted my time but attempted to gaslight me. I have hyperparathyroidism. I had surgery to removed 2 adenomas on 3/24/21. Over the past two years, my calcium has occasionally crept up over 10, again. My PTH is back where it was pre-surgery in 2020-2021. My eGFR, all measured in the MGB system with the same calculation, has gone from low 100s to 73 from January of this year to July. I am having frequent UTIs, one noted with micro hematuria. I am thinking secondary hyperparathyroidism. I shared all this with Dr. Irwig. I gathered all of my labs pre- & post-surgery, my surgeon's records, procedures documenting my bone loss, and made sure they got to Dr. Irwig in advance of my appointment. Yet it was very clear that Dr. Irwig hadn't even glanced at them and wasn't all that familiar with hyperparathyroidism. His final answer? "Your calcium is fine. Test yearly."

He is a bully & will attempt to push his own agenda onto you... regardless of your needs. He talks right over you and doesn't listen. Rude & arrogant.

Be careful, he will push transitioning onto you, even when you try to say no.

Dr. Irwig has probably the best bedside manner of any physician I have ever been to. He was thorough, answered all questions I had. I even had a question that came up and I emailed him through the health portal, and promptly received an answer that same day. I am just sad that it looks like he has moved to Boston and is no longer in the DC area (GW). Great Endocrinologist who I would highly recommend to anyone.

Dr Irwig is the best endocrinologist. He understands and is an ally for the trans community.

Dr. has twice cancelled appointments for no reason. I showed up to one appointment only to be told that the doctor had cancelled my appointment (same day cancellation) and not notified me.

I was very impressed with Dr. Irwig; he seemed very knowledgeable and personable. You have to plan for a little longer Dr. visits because its a teaching facility. But everyone was very nice and professional. One thing I did like was that he is connected to all my other Dr. & health records at GWU. Additionally other doctors have you do labs a week before your visit but they have the capability to do the A1C during my Dr. visit. So all in all I give Dr. Irwig high marks; I was very satisfied

Michael is an excellent Endocrinologist. He reads a lot and is very updated about the new development in the field. The best part is his treatment plan - he never over prescribes the medication. He spends a lot of time - listening, probing and giving his advice.

Very by the book - isn't particularly open to working with patient to truly figure out the best course of medication and treatment. If patient blood tests say normal, even if patient has listed out why s/he doesn't feel normal, then patient is out of luck and is shooed out of the door with a reminder for another appointment.
