I am a physician who was in serious trouble due to my behavior and was required to be evaluated by the Positive Sobriety Institute to determine my fitness to practice. I came to town fully informed that I would go through a multidisciplinary evaluation. As this was a fitness for duty evaluation all factors were taken into account and incorporated into Dr Mele's neuropsychological evaluation results. The test was conducted in a standard Dr's office. As I did not pass the NP evaluation it was requested by PSI that I return for another evaluation. I did not do so but passed a similar evaluation at Mayo Clinic. Despite "passing" at Mayo I am now on disability and regret my criticism of Dr Mele's evaluation, as the results of his evaluation were later requested to gain said disability status. Healthgrades clearly did not vet my initial comments regarding Dr. Mele

Communicate a visit to him. My insurance states they will cover everything other than $15 copay. He advises me that I’ll have to pay out of pocket anywhere between $1500-$2500 out of pocket.

While at Positive Sobriety Institute I was to have a Neuropsychology test. Dr. Mele, although seemingly a nice fellow, did a substandard job administering the test. The test was done in a very noisy and distracting setting. Additionally, Dr. Mele did not inform me that I was even to have the test which is the standard. A patient should know well ahead of time and a test of this sort should not be sprung on someone. I didn't even have lunch that day. This may have been more the fault of Positive Sobriety Institute yet Dr. Mele should have rescheduled the test. Finally, Neuropsychologists are aware that test results are dependent upon one's current condition and it is advised that when one is in a state of depression the test results are not reliable. This was not considered in my situation. PSI tried to force me to return to Chicago to repeat the test at a cost of $1,400. The trip alone is a nine hour drive from home. Eventually I retested at Mayo Clinic and passed easily.
